Center for Health & Healing

The Center for Health and Healing provides substance use and mental health counseling and case management services delivered by trained and skilled staff, based on the strengths, needs, abilities, and preferences of each individual. Programs for children and adolescents include behavioral health services tailored to the needs and preferences of youth and delivered in age-appropriate, supportive settings. Length of services is based on individual needs.

The Center for Health and Healing provides counseling and case management for children, adolescents, adults, caregivers, and their families. Using a two-generational approach, the program aims to address the interconnected needs of caregivers and youth simultaneously to break cycles of trauma, improve family stability, and promote long-term emotional well-being. Services are grounded in evidence-based practices, trauma-informed care principles, and comprehensive case management to ensure holistic support and sustainable outcomes.

Counseling

Individual counseling services focused on emotional wellbeing, coping skills, stress management, trauma, substance use, behavioral concerns, life transitions, grief, and family related struggles.

Case Management

Short term support connecting individuals and families to community resources, insurance assistance, housing supports, and other basic needs services.

Our Intake Process

Step 1: Call Our Office

Step 2: Get You Connected

Sessions occur at our main office at 624 Xenia Avenue. East End has multiple funding sources, and we do not turn individuals away for the inability to pay for services. Please bring your insurance and identification documents to your initial assessment. For minors receiving services, documentation verifying legal guardianship is required prior to treatment.

Step 3: Get an Assessment

The initial session consists of a mental health assessment to determine your needs within what our program offers. This session will last approximately 45-55 minutes.

Step 4: The Program Offerings

Based on your assessment results, the appropriate level of care will be recommended, which could include counseling, case management, or both services combined. If following the initial assessment, it is determined that a higher level of care or a more appropriate referral is necessary, we will provide support with this recommendation.
